Once you have logged-in you should see image like the one below. If there are any missing or errant information in the fields in patient information area, please click "Edit Patient" and fill in or correct the information.

As seen above one should see a series of blank clinical picture frames just to the right and below the patient information. Please upload pictures like those in the following examples. The Aux picture slots are for scars of previous surgeries or points of interest for the surgery. The portrait picture is for a passport style picture. Lumbar patients should upload pictures of the abdomen (front and side) from the pelvis to the ribs. Such images are necessary for Dr. Bertagnoli to judge entry.

Simple pictures from a digitial camera is all that is needed. Very large pictures (over 2 mp) are not necessary.



Next as seen in the image below, the patient should answer the questionnaires entitled patient history, General, NDI (ODI for lumbar patients) and the SF36. The title will turn green when it is completed.

Just click on the red text to open the form/questionnaire. The patient should answer all the questions on the questionnaire or else it will not save. There is an exception for a question on sex life on the ODI.

Next, please upload any medical reports you would like to send. These can be in any common format, but we prefer PDF. Please do not make one page per file of the report. We would like all the pages of a report in one file. You can upload as many reports as you like. Just left-click on "Medical/Clinical Reports" as many times as you need.



Next, we must prepare the image CDs for upload.

You must zip/compress all the files and folders on the CD into a single file your hard drive and then upload that file. You only need to upload the image CDs you have at the moment.

STOP! Please do not proceed until you have compressed/zipped your CDs!

PLEASE CLICK ON THE APPROPRIATE CATEGORY FOR THE UPLOAD. MRIs SHOULD GO IN THE MRI FOLDER AND X-RAYs IN THE X-RAY FOLDER AND SO FORTH If others are required, they will be requested of you. Usually we start with an MRI, but if you have a CT or Myelogram or X-rays please upload them also. Free zip/compression software at http://www.izarc.org. Please do not send zipx files.

Now, simple left-click on the type of image you wish to upload and a file upload box will appear on your computer. Then select the file you have zipped.



A log of all your medical report and images uploads is made and shown to you.

Now you are done. Just log out.
